You can become a Covenant Affiliated Church for $350 annually mailed to LMF P.O. Box 1004, LaGrange GA 30241. Covenant Churches agree to tithe 10% of monthly income to LMF, Inc. (this can be done through the Covenant Giving button within the LMFinc.com website). Upon completing the form below, you will be required to scan and email to memberservices@lmfinc.com a copy of your

1) Articles of Incorporation

2) Corporate Bylaws

3) 501c3 determination letter if applicable) and

4) State in the message box below thatyou agree to be in a relationship of

accountability with LMF.
